ABOUT THE ROLE

We are seeking a compassionate and skilled Physiotherapist to join our Palliative and Transitional Care Team. This is a unique opportunity to make a meaningful difference in the lives of individuals and families.

Transitional Care Program provides a low-intensity, time limited, and coordinated interprofessional restorative model of care to adult clients who have experienced a recent decline in independent function. The program strives to support clients in returning home, either independently or with appropriate community-based resources.

Key Responsibilities:

• Provide physiotherapy services to clients in palliative care and transitional settings, including those awaiting alternate levels of care such as long-term care, rehabilitation, or supportive housing
• Manage a high-volume and diverse caseload with efficiency and professionalism, ensuring timely and appropriate care for all clients.
• Collaborate effectively within an interdisciplinary team, actively participating in daily huddles, patient rounds, and family conferences to ensure coordinated, people-centered care
• Supervise and delegate appropriate tasks to Rehabilitation Assistants, ensuring quality of care and alignment with physiotherapy goals and treatment plans
• Actively participate in quality improvement initiatives, including the development and implementation of best practices aimed at enhancing patient outcomes and service delivery

WHAT YOU BRING

Education/Experience:

• A degree or equivalent from an accredited Physiotherapy program
• Current registration with the College of Physiotherapists of Ontario (CPO) and thorough knowledge of its Code of Ethics and Standards of Practice

 

Skills/Abilities:

• Demonstrates a deep understanding of goals of care, showing empathy, compassion, and respect when working with individuals and families navigating life-limiting illnesses
• An organized professional skilled in triaging and prioritizing coverage to ensure optimal allocation of resources on any given day
• Ability to manage a clinical practice in a health care setting
• Able to adapt to different client needs and provide care in a variety of hospital settings
• Ability to complete analysis, using clinical knowledge, skill and judgement to synthesize assessment information
• Ability to communicate effectively and concisely, both orally and in writing, as well as able to convey complex information with clarity and sensitivity
• Ability to work independently, interdependently, and collaboratively within a model of interprofessional care
• Ability to develop and maintain respectful partnerships
• Commitment to clinical excellence and leadership in Rehabilitative care
• Demonstrated commitment to People-Centred Care approach and principles
• Ability to demonstrate reflective practice
• Strong project/time management, analytical/critical thinking, problem-solving, and decision-making skills
• Ability to use independent judgment and maintain confidentiality of records, actions, and information
• Excellent assessment and intervention skills are required
• Demonstrated regular attendance at work

 

Asset:

• Membership in Canadian Physiotherapy Association (CPA) and Ontario Physiotherapy Association (OPA) are strongly encouraged
• ADP Authorizer status or willingness to obtain authorization is considered an asset.

 

Conditions of Employment:

• This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
